Laptop Reinventory for Students, Faculty & Instructional Technology Fellows

As part of the New York State Mandated Reinventory program, all students in the classes of 2011 and 2012 (juniors and sophomores), faculty members, and ITFs must complete this form before November 15, 2009. This form is to confirm that you still have your Macaulay laptop as it was issued to you.

If you are a freshman or a senior (class of 2013 or 2010), please do not complete this form; it does not apply to you.

Where do I find the asset tag number and the serial number?(click for larger image)

Please complete all information carefully and accurately. Do not confuse the letter "S" with the number 5, or a zero (0) with the letter "O" There should be no letter "O" in any serial number.
