Macaulay Student Name Change Form

Please use this form to indicate your preferred name and the systems that you’d like your preferred name to be changed within Macaulay Central. Once this form is submitted, notifications will be sent to the operators of the systems that you checked off, who will make this change and confirm with you that this change has been made. Please allow 1-2 weeks for these changes to be processed.

Please refer to the Macaulay Student Name Change Policies & Checklist to find more information about changing your preferred name on your home campus and other Macaulay systems where you have access to make this change on your own.
